1. Express Assumption of Risk
I understand that Brazilian Jiu-Jitsu is a contact sport. Training involves intense physical contact, submissions, choking, joint locks, throws, and takedowns. I acknowledge that inherent risks include, but are not limited to:
- Minor injuries like scrapes, bruises, cuts, and sprains.
- Major injuries like broken bones, joint dislocations, concussions, and torn ligaments.
- Catastrophic injuries including paralysis or death.
- Exposure to communicable diseases, bacterial infections, and viruses.
I voluntarily assume all known and unknown risks associated with my participation.
2. Release and Waiver of Liability
I hereby release, waive, and forever discharge Relson Gracie Jiu-Jitsu of Charleston, its owners, instructors, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of negligence. This includes any injury, illness, or property damage suffered during training, competition, or while on the premises.
3. Membership Cancellation Policy (30-Day Notice)
I understand and agree to the following billing and cancellation terms:
- All membership cancellations require a written 30-day notice.
- Cancellation requests must be submitted via email or official academy form.
- Any billing cycle occurring within 30 days of the notice will be charged as scheduled.
- No partial refunds or pro-rated refunds will be issued for mid-cycle cancellations.
- Access to facilities and classes continues through the end of the final paid period.
4. Medical Affirmation and Authorization
I certify that I am physically fit to participate in strenuous martial arts training. I do not suffer from any medical condition that would make my participation unsafe. In the event of an emergency, I authorize the staff to secure medical treatment on my behalf. I accept full financial responsibility for any medical expenses incurred.
5. Photo and Video Release
I grant Relson Gracie Jiu-Jitsu of Charleston permission to use photographs, video recordings, or digital media of my training for promotional, marketing, and educational purposes without compensation.
6. Governing Law
This agreement is governed by the laws of the State of South Carolina. If any portion of this waiver is found to be invalid or unenforceable, the remaining sections will continue in full legal force.
